You have been allocated a randomly generated number that allows us to protect your identity fully as you report on your knee symptoms and function.
There are 2 questionnaires that we ask you fill out, the Knee Outcome Survey Activities of Daily Living Scale (KOSADLS) and the Lysholm Knee Scoring Scale (LKSS).
You will be filling out each scale twice.
The first time represents your best recollection of what your knee was like before the operation. For each question, try to remember your daily activities then, and how your knee affected you. We acknowledge that the such information is best collected when you are first seen, but for many of you, the operation was many years ago, before this study was envisioned. So do the best that you can.
The second time for each scale represents what your knee is like now.
